  • helping participants and their plan nominee to understand their NDIS plan
  • educate participant and plan nominee to use the NDIS portal
  • identify service providers to work with participants NDIS plan, goals and within the participants allocated budget
  • support participants to aim to live as independently as possible within their community using a collaborative approach to care needs
  • write reports required for use with the NDIS and gather relevant information to send to NDIS
  • attend and advocate at planning meetings for future plans and relevant funding

What is support coordination?

Support Coordinators assist you to understand, implement your plan and ensure you are being connected to the right providers to meet your goals. They focus on supporting you to build skills. Their role is to assist in planning and ensure you are optimising your plan. In most cases, support coordinators will attend plan review meetings with their participants.
