Hospital Discharge Support Coordination
Our team of support coordinators have varying experience, including working in and with health systems. We work with participants to have timely discharge and transition back home through necessary support and coordination.
Our support coordinators work closely with hospital teams and the NDIS Health Liaison Officer to thoroughly plan for a timely and efficient discharge. We can support participants to return home or into new living arrangements such as Supported Residential Services, Specialised Disability Accommodation, Medium Term Accommodation and Supported Independent Living.
Our support coordinators can:
- Attend hospital planning meetings
- Attend hospital case conferences
- Arrange handovers between the hospital team and your community services
- Arrange new supports identified because of your hospital admission
- Arrange equipment as recommended by your hospital team
- Monitor and review your transition back into the community
